We are seeking an Emergency Department Nurse Case Manager to:

  • Be part of a dynamic multidisciplinary team and help facilitate interdisciplinary care across the continuum

  • Use problem-solving skills to connect patients with the right care and link them to the resources they need

  • Be an advocate for patients

  • Work face-to-face and on the phone with patients and their families


 

Education:

Minimum - BSN degree
Preferred - MS degree
 
Work Experience:
Minimum - Three years relevant clinical nursing experience.
Preferred - Previous Emergency Department or Case Management experience
 
Licenses and Certifications:
Minimum - RN licensed in the state of Wisconsin
Preferred - Emergency Department or Nurse Case Manager Certification
 
Required Skills, Knowledge, and Abilities:
  • Excellent interpersonal communication, problem solving and conflict resolution skills.
  • Computer skills in word processing, data base management, and spreadsheet desirable.
Physical Requirements:
Sedentary: Ability to lift up to 10 pounds maximum and occasionally lifting and/or carrying such articles as dockets, ledgers and small tools.  Although a sedentary job is defined as one, which involves sitting, a certain amount of walking and standing is often necessary in carrying out job duties.  Jobs are sedentary if walking and standing are required only occasionally and other sedentary criteria are met.
 
Work Schedule:
  • 90% FTE, 36 hours per week.
  • Day/Evening/Night, 12 hour shifts with the hours of 7:00 am - 7:00 pm and 7:00 pm - 7:00 am.
  • Weekend rotation
  • Hours may vary based on the operational needs of the department.
