Advanced Practice Provider: Family Medicine

UW Health in northern Illinois seeks an Advanced Practice Provider (APRN or PA-C) to join our growing primary care team in Rockford, Ill. We are looking for dedicated candidates to provide our patients with high-quality treatment and embody our mission:

A variety of positions in primary care are available as we focus on improving access and experience for our patients. Positions include, but are not limited to, the following:

  • Collaborating closely with a Family Medicine or Internal Medicine physician in supporting their panel
  • Dermatology-focused care with our growing team
  • Working in our float pool of experienced providers in a variety of clinic settings
  • Managing patients in our Welcome Clinic, providing transitional care for patients before they establish with a primary care provider

Your clinical support includes the following:

  • The region’s largest and most-preferred medical group and health system
  • Full-service academic hospital support including the SwedishAmerican Heart Hospital and the SwedishAmerican Women’s and Children’s Hospital
  • Excellent tertiary specialty support from University of Wisconsin School of Medicine and Public Health

Qualifications

  • Master’s degree from accredited PA program or completion of a master’s level advanced practice nurse program 
  • Active PA license in State of Illinois and Active DEA license or the ability to apply for such license prior to or upon hire
