UW Health Northern Illinois seeks an Advance Practice Provider (APRN or PA-C) to join our Same Day Care initiative at UW Health SwedishAmerican in Rockford, Ill.
With a focus on providing same-day access to patients within our system, the experienced Advance Practice Provider is responsible for seeing patients at clinics throughout the region. Appointments include acute issues, chronic issues, mental health assessments, medication re-checks and physicals (school, sports, employment). The provider is expected to be an excellent communicator, both in educating patients and family members and communicating care plan to multi-disciplinary team.
Your clinical support includes the following:
- Full-time practice within the hours of your assigned clinic
- Requirement of 12 Saturdays per year (3 hours on Saturdays)
- Telemedicine opportunities
Qualifications:
- Master’s degree from an accredited PA program or completion of a master’s level advanced practice nurse program..
- Active APRN or PA license in the State of Illinois and Active DEA license or the ability to apply for a such license prior to or upon hire.
- Experience and proven effectiveness as practicing clinician
