Advanced Practice Provider: Psychiatric Crisis Nurse Practitioner (PMHNP)

Position highlights

UW Health in northern Illinois is seeking a dedicated and compassionate Psychiatric Mental Health Nurse Practitioner (PMHNP) to join our team in providing overnight care for patients at our SwedishAmerican Hospital. This role is integral to providing effective psychiatric care and crisis intervention in-house at our 340-bed hospital located in Rockford, Ill.

As a provider with our Center for Mental Health Department, you will offer comprehensive support to patients in crisis, ensuring they receive the best possible care and treatment at the only facility with in-patient psychiatric services in the northern Illinois region.

Key responsibilities

  • Provide comprehensive psychiatric assessments and crisis intervention from 7 p.m.–7 a.m. for patients in the Emergency Department (ED) and the 42-bed inpatient mental health unit (comprised of 26 adult beds and 16 child and adolescent beds), as well as medical consults throughout the hospital.
  • Additional support from Assessment and Referral (A&R) available during the night to assist with patient evaluations and admissions
  • Develop and implement individualized treatment plans for patients in acute psychiatric crises
  • Collaborate with ED staff, Center for Mental Health staff and other health care professionals to ensure integrated and effective patient care
  • Perform psychiatric evaluations and manage medication regimens for patients
  • Conduct follow-up assessments and adjust treatment plans as necessary
  • Maintain accurate and up-to-date medical records and documentation
  • Participate in multidisciplinary team meetings and contribute to case discussions
  • Ensure continuity of care by coordinating with daytime psychiatric providers and other hospital staff

Qualifications

  • Master’s degree from an accredited Psychiatric Mental Health Nurse Practitioner program
  • Current licensure as a PMHNP in Illinois, or the ability to apply upon program completion
  • Minimum of one year of experience working as a PMHNP in an in-patient setting, preferably with crisis intervention experience.
  • Strong clinical, analytical and problem-solving skills
  • Excellent communication and interpersonal skills
  • Ability to work independently and collaboratively in a fast-paced and dynamic nocturnal environment
