LCSW/LCPC

Position highlights

UW Health Northern Illinois seeks an experienced LCSW/LCPC to provide behavioral health services in our primary care ambulatory clinics to enhance psychosocial functioning and continuity of care for patients in the greater Rockford, Ill., area.

Working collaboratively with primary care providers and the care coordination team, you will provide initial evaluations, develop treatment plans, conduct individual, group, couple and/or family therapy, and advocate for patients dealing with personal issues including mental illness, addiction and abuse. You will also provide assistance with overall program development, community services, training and education in behavioral and mental health.

Qualifications

  • Master’s preferred, with one year of clinical social work required.
