Eligibility requirements

The Upper Extremity Physical Therapy Fellowship at UW Health is a full-time, 14-month program.

Eligibility

To be eligible for the Upper Extremity Physical Therapy Fellowship you must:

  • Have a valid physical therapy license in both Wisconsin and Florida
  • Be a licensed athletic trainer, certified emergency responder or licensed EMT

To obtain information on licensing, contact the Wisconsin Department of Licensing or the Florida Board of Physical Therapy.

Application process

  • Cover letter detailing the reason for an application to the fellowship program
  • Statement letter of goals and objectives for fellowship area of interest
  • Current resume/CV
  • Three letters of reference including one from a former employer
  • Submit all required materials here

Application dates

  • Application period opens Feb. 15, 2024
  • The application deadline is March 30, 2024
  • Remote interviews will occur April 1 – April 30, 2024
  • Position offer date is May 1, 2024
